Community Support Coordinators CR19 MAST – Community Support Coordinators x 2 Based in Bradford Royal Infirmary and Airedale Hospital 37 hours per week fixed term to March 2022 (will include one weekend day per week) £22,459 – £24,708(Salary scale quoted for 37 hours per week, applicants appointed to bottom of pay band) MAST is a partner project to support inpatients and patients that are being discharged from hospital that then need further support when at home. The purpose of the role is to ensure people receive ongoing support in the community, to maintain their health, wellbeing and independence. Candidates should have experience of Health or Social work, and experience of working in a busy hospital environment and the ability to work 1to 1 with individuals and to develop support plans for vulnerable people.
