Department: HEAD OFFICE Reporting to: Registered Manager Responsible for: Providing high quality care to service users and others who require care and resources in their homes or in a Residential Care Home. Purpose of job:Your purpose as the Care coordinator is to ensure effective coordination of care allocation and delivery by working in partnership with the Field Care Supervisors.To ensure that rotas are communicated to care and support staff in a timely manner and ensure that rota queries are dealt with promptly. Role:To work as part of an effective team in implementing and evaluating the aims, objectives, policies and procedures of the company to ensure an efficient and effective service delivery.To assist in the smooth running of the office and its multi-disciplined service activities.To meet agreed targets in accordance with the aims and objectives of the Company by developing and maintaining good business relations and by ensuring all staff work together as a team.To assist the Registered Manager to organize the day to day running of the service.To Proof reading reports from Contact Supervisors.To update care referrals and assigning or matching the correct Contact Supervisor to the ClientTo carry out initial assessments of new services.To visit Service Users and to make reviews of the written Care Plans, examine the written records of medication administration, finance and daily log notes kept by Staff as required by the Registered Manager.To produce detailed work schedules for the Care Workers and providing both written and verbal instructions/guidance to them. To participate in the 'out of hours' call rota.To carry out any other tasks required by the Registered Manager that are commensurate with the level of responsibility. Responsibilities Main Areas of Accountability: Assist the Registered Care Manager in all aspects of care management.Ensure that care is provided according to all relevant policies, procedures and regulations.Maintain accurate records of care workers and Service User’s. Record relevant details of all new Service User’s and care workers according to Company policy and keep all records updated and in accordance with Data Protection regulations.Observe legislative requirements according to Working Time Regulations.Observe service user and care worker confidentiality.Assist Registered Care Manager in the accurate and appropriate matching of care workers to service users.Assist with the recruitment of care staff and sourcing new business for care workers as well as liaising with social workers and home managers.Undertake Risk Assessments in service user’s home.Proof read reports written by the Contact Supervisors before e-mailing to Social ServicesConsult with family members and other approved persons to ensure that care is on-going and that all client needs are identified and met.Promote and ensure effective communication and full participation between care staff and service users at all times.Produce periodic data for finance department.Assist with problems that may arise between care workers and service users effectively on a daily basis.Observe Company Policy in relation to issues surrounding confidentiality.Communicate and liaise with all office personnel, social workers, service users and care workers.Assist with ‘On-Call’ duties during out of office hours including weekends and bank holidays.Ensure that care workers are trained to administer the required care.Cover emergency work for care workers due to illness or unforeseen circumstances.Mentor new care workers, offering support, advice and encouragement.Conduct care worker’s supervision, identifying training needs, assessments and regular monthly meetings.Coordinate staff annual leave and any relevant HR duties.Submit monthly reports to the Registered Manager. Attend training courses as required and identified by the Register Manager. Uphold standards of the Care Standards Acts. Uphold the organizations equal opportunities policy at all times.Make recommendations for changes and improvements as required.Any other job assigned from time time relating to Care services, Care Workers and Service Users. Typical work activities - Some of your main responsibilities will include: Planning, allocating and evaluating the workload of the care/support workers to ensure a safe and effective service delivery. Particular attention should be paid to providing continuity of care/support to individual service users.Ensuring that any sickness/absence is adequately covered.Participate in the recruitment of care workers in line with company policies and guidance and ensuring that all recruitment checks are satisfactorily completed.To work with other Care Coordinators as required to ensure a smooth changeover is effected where necessary between areas of responsibility.
