Barnardo's Rochdale Service is looking to recruit two highly skilled and experienced practitioners as keyworkers to support children and young people with Autism Spectrum Condition and/or Learning Disabilities and their families.While professional experience and qualifications are key for this role, make sure to check you have the preferable soft skills before applying if required.This new initiative is part of NHS England's improvement plan to develop and implement new keyworker models.As a children and young person's named keyworker you will work within families who are experiencing significant difficulties and need wrap around support to cope and beyond that to thrive.The children and young people you support will come via the dynamic support database and be at amber or red and as such will require individualised co-ordinated support in a timely manner.You require significant experience in supporting children with Autism Spectrum Condition and Learning Disabilities to succeed in this role.This role combines the need to be a highly motivated skilled practitioner confident in delivering support within the family home with the ability and confidence to attend and chair meetings involving the family, education , health, social care and voluntary sector colleagues.You will work in partnership at all levels to achieve the best outcomes and respectfully address at a strategic level any barriers to successful outcomes for families.If you have the drive, passion and experience for this exciting new role then we are interested in your application .Please ensure you read the additional information attached to the generic job description as shortlisting will be undertaken in the context of this.
