We are looking for a talented candidate who can utilise their previous knowledge of the domiciliary industry. You will need to possesses excellent organisation and coordination skills to effectively harmonize with the team to deliver a first class service to all clients and customers.  Responsibilities of the Care Team Coordinator:  The team you will be joining are fundamental in ensuring that my clients customers receive an effective service either in person or on the phone.  This is an energetic, friendly and fast-paced environment. You will be required to hold the on call phone, and respond appropriately to rota concerns and emergencies.  Where the need arises you may be required to undertake a caring role in the community.  Requirements to become the Care Team Coordinator: •  The ability to organise and coordinate the daily care visits using CM2000 (training provided) •  To be able to observe live data, consolidate and rectify any issues with live Call Monitoring. •  Hold a good working understanding of computer based programs i.e. care planning systems, excel, word, outlook etc. • Good organisational and timekeeping skills is a must. • You need to be a ‘Good’ Communicator, and understand the need for effective and timely communication. • The ability to hold and maintain a good working knowledge of the issues faced by carers and individuals who receive care services. • Must hold or be working towards Health and Social Care Diploma level 3 or equivalent. • Be able to manage good handovers between teams, which will involve attending a virtual handover meeting the day before your rota starts and the day before it ends, attendees are Coordinators, Field Care Supervisors, Administrators and Managers. • The ability to work under pressure, and make good decisions ‘on the spot’. • Understand the need to ask for support when required and in good time.  A driving license is preferred however not essential Hours of work are Approx. 44 hours a week working split shifts 7-2 and 4-10 on a four day on four day off basis.  The majority of your hours would be spent in the office, particularly between 9 and 2 however under the current coronavirus regulations there is some ability to work from home with this role.   This is a fantastic opportunity to join a well-recognised company during a key period.
